Birth and Death Certificates

As a service to the public, the Caddo Clerk's Office has obtained access to the State Registrar in order to issue certified copies of Birth Certificates and Death Certificates.

Please note that we are only able to make copies. If you need information on registering a birth or changing incorrect information, please contact the State of Louisiana Department of Vital Records.

The fee is $34.00 for a Birth Certificate, $48.00 for a Birth Certificate and Birth Card (sold as a pair only - Act 270), and $26.00 for a Death Certificate, in cash or credit card only. Fees are mandated by the State according to Act 390. Copies may be purchased between the hours of 8:30 a.m. and 4 p.m. at the Clerk's Office. After 4 p.m., you may have to return the next day to receive your certificate depending on the availability of the State's computer system.

Clerks Offices can only issue Death Certificates registered in Louisiana from July 9, 2012 to the present.

If you live outside of Louisiana or are unable to visit the Clerk's Office to obtain your copy, the State of Louisiana has a partnership with VitalChek so you may obtain a copy online.

Who may purchase a certified copy of a Louisiana Birth Certificate

  • Registrant (person named on certificate)

  • Current legal spouse of person named on certificate

  • Mother or father of person named on certificate

  • Adult child of person named on certificate

  • Brother or Sister of person named on certificate

  • Grandparent of person named on certificate

  • Grandchild of person named on certificate

  • An attorney representing one of the above with a signed contract of representation or authorization

  • Legal Guardian of person named on certificate. In this case, you must provide a Judgment of Custody. Custody by mandate or provisional custody from a notary, or transfer of custody witnessed by a notary will not be accepted.

Who may purchase a certified copy of a Louisiana Death Certificate

  • Current legal spouse of person named on certificate

  • Mother or father of person named on certificate

  • Adult child of person named on certificate

  • Brother or Sister of person named on certificate

  • Grandparent of person named on certificate

  • Grandchild of person named on certificate

  • An attorney representing one of the above with a signed contract of representation or authorization

  • Legal Guardian of person named on certificate


ID Requirements for Birth or Death Certificates

One Primary Document or Two Secondary Documents

Primary Documents (any one of these)

  • Current state issued driver's license that displays a photograph which clearly identifies the applicant.

  • Current state issued picture ID that displays a photograph which clearly identifies the applicant.

  • Current U.S. military ID that displays a photograph which clearly identifies the applicant.

  • Current U.S.-issued or Foreign-issued Passport

  • U.S. Certificate of Naturalization (Form N-550, N-570 or N-578)

  • U.S. Certificate of Citizenship (Form N-560 or N-578)

  • Students (high school or younger) a current school yearbook or a current school ID with a photograph which clearly identifies the applicant.

Secondary Documents (any two of these)

  • Current student picture ID from a college or university when accompanied by a 100% fee paid receipt for the current semester (counts as two documents).

  • A W-2 form issued within the last two years plus an original signed Social Security Card. The Social Security numbers must match (counts as two documents).

  • Original adoption papers

  • Official certified deeds or title to property

  • Certificate of vehicle title

  • Insurance policy which shows the name of the applicant (Health, Home, Life or Auto)

  • Payroll stub (cannot be handwritten) showing the name and social security number of the applicant

  • Current U.S. military dependent ID that displays a photograph which clearly identifies the applicant

  • Original DD-214 Military Discharge document which clearly identifies the applicant

  • Voter's Registration application (certified true copy)
